A boy 8 years old presented to the emergency department with 24 hours of right lower quadrant pain with anorexia and nausea. He also reported mild fever. On exam,  vital signs were normal, temperature of 38.1 ⁰C. Abdominal exam revealed a soft abdomen with right lower quadrant tenderness with voluntary guarding and mild rebound tenderness. The WBC was 23.3 x 10³ /μL with 83.1% neutrophils.
